Request Transcripts
We will be happy to provide you with a copy of your most recent transcript. When school is in session, please allow at least five business days for your request to be processed. The cost for each transcript transmission is $5.00.
Please note that “official” transcripts must be transmitted directly from Holy Names High School to the requesting institution. Transcripts transmitted to graduates are "unofficial" and are not signed.

REQUEST BY MAIL: Download and print the transcript request form below. Please fill out the form as completely as possible in order to ensure timely and accurate mailing of your transcript. Enclose the completed form with a check for $5.00 per transcript. Please make the check to Holy Names High School and mail to:
